PACT EARLY HEAD START & HEAD START SUMMARY
What is this?
PACT’s Early Head Start provides center/home-based child care services for low-income families with children under the age of 3 years old. PACT’s Head Start provides child care services for low-income families with children between the age of 3 to 5 years old. Part-day (4-5 hours/day)/Full-day (6 hours/day) services are free, but extended day services (8-10 hours/day) will require a fee. Applications are accepted year-round.

Who qualifies?
- Low-income families with income levels 130% or below Federal Poverty Level - Early Head Start: - Children ages 3 years old or below OR prenatal mothers 
 - Head Start: - Children ages 3 to 5 years old. Needs to turn 3 years old by July 31st and not eligible for kindergarten. 
 
What to provide
- Basic contact information (phone number, email address) 
- Completed application form 
- Provide of one of the following documents: 
- Proof of income for the last 12 months (pay stubs, tax return) 
- SNAP, TANF 
- Foster parent status confirmation 
- Copy of child’s birth certificate 
- Child’s current immunization records 
- Child’s current physical examination form (Form 14) 
- Child’s Tuberculosis (TB) clearance form (within past 12 months for child at least age of one years old) 
- Verification of 30 or more hours per week of the following activities: employment, job training, or school (IF ENROLLING FOR EXTENDED DAY CLASSES) 
- Pregnancy verification form (FOR PRENATAL SERVICES ONLY) 
Application process
- If clients have already applied in the past or would like to get assistance in applying, call (808) 842-5996 
- To apply via mail, clients must request an application form from the nearest PACT office (KPT) 
- Fill out parent/guardian/prenatal mom basic contact information 
- Enter family information (number of parents/guardians, relationship to child/applicant, primary language, number of family members, any benefits received, etc.) 
- Enter secondary adult information (if applicable) 
- Enter child/applicant information 
- Sign and date application 
- Either email application to hs-ehs@pacthawaii.org OR drop off/mail application form with copies of the required documents to the following address: 
- 1300 Halona Street, Honolulu, HI 96817 
- Receive an email/call after submitting application 
- Expect to receive a call from a PACT representative within 7-10 business days for an interview and request of required documents 
- To apply online, clients must go to https://pacthawaii.org/our-work/early-headstart-programs/early-childhood-education/ : 
- Scroll down until they reach “APPLY ONLINE TODAY” and click to start the application. 
- Enter the parent/guardian basic contact information 
- Enter family information (number of parents/guardians, relationship to child/applicant, primary language, number of family members, any benefits received, etc.) 
- Enter child/applicant information 
- Choose either Early Head Start OR Head Start 
- Choose a 1st location preference and a 2nd location preference based on availability. 
- Enter any sibling/non-applicant information if there are any related to the applicant. 
- Check off verification box and click “Save and Continue” to submit application. 
- Right after submitting application, upload required documents on the following page 
- Receive an email after submitting application
