Home Visits and Geriatric Medicine Consultations

Kokua Kalihi Valley Health Center is increasing its services to the frail elderly, homebound and bed-bound persons in our community. In order to improve access to medical care we are pleased to announce the availability of physician home visits and geriatric medicine consultations.

We have two board certified Geriatric Medicine Specialists Dr. Renato Reyes and Dr. Ritabelle Fernandes.

Dr. Reyes attended the Cebu Institute of Medicine in Cebu City, Philippines and is fluent in Tagalog, Ilocano, Cebuano and Ilongo.

Dr. Fernandes graduated from Seth G.S. Medical College, University of Bombay, India. Her special interests are Palliative Medicine and End-of-Life Care.

KKV will provide the following services:

Home based primary care
Geriatric medicine consultations for frail elderly, or seniors with multiple health problems
End-of-Life care for persons with terminal illness
Collaboration with case managers, health maintenance groups, caregiver support groups etc.
